Tilmon is a foul machine, we all know that. However, he also has the reputation of being a foul machine and is likely being targeted by the referees. He is the victim of at least one "bad" call every game and is not getting any 50/50 calls.
Cuonzo rarely yells at the referees. I think he's too nice a guy and has publicly stated his reluctance to berate them, saying they are "just guys trying to do their job." It's hard not to respect this.
However, I think he needs to have a Gundy moment when he pretends to lose his temper defending his player. It needs to be an issue, big enough for a league fine. It needs to be talked about by the press, amplified by the fans, and something the referees can't avoid. I would suggest doing it after a game where Tilman fouls out (uhhh....so, any game) and before a big home game where the fans can yell and scream. Maybe even a montage of bad calls submitted to the media.
I think it's part of the job as head coach to use the media if it can help.
